Occupational Health Skin Surveillance Tools

It has long been established in dermatology that the residual moisture level in the surface layer of the skin (the stratum corneum) is an indicator of general skin condition. Instruments to measure the skin's residual moisture content have been available for dermatological research since the 1980s, but not in a form that is best suited for use in the occupational health environment. The Skin Hydration Monitor EDS10 measures skin moisture levels and has been designed specifically for use in the occupational health environment. Measurement of skin hydration is simple, non-invasive, quick and intrinsically safe. The instrument is simply placed on the skin, depressed and the resultant number read from the screen. This indicates whether the skin is in normal or damaged condition and gives an indication of the severity of the damage.

The EDS10 Skin Hydration Monitor can detect damage due to chemical exposure that is not visible to the naked eye. This allows the user to identify whose skin is being damaged and the extent of the damage long before this becomes visible. Action can then be taken to prevent further damage and to restore the skin to its normal condition. The instrument can subsequently be used to establish whether the action taken is achieving the desired result. So instead of simply waiting until the skin damage escalates into a clinically relevant dermatitis it is now possible to be pro-active in the same way as is done with lung function testing and audiometry.

Safe, effective use of gloves as protection against chemical and biological hazards

It is inevitable that in many organisations there will be a need to provide gloves as protection against chemical and biological hazards. If correctly selected and used they can provide effective protection for your workforce. Unfortunately, unless the system ensures they are selected and used correctly the use of gloves may actually increase the effect of chemical contact, resulting eventually in contact dermatitis, contact urticaria or damage to internal organs.

Ensuring the correct gloves are selected and used within their real performance limitations is far from as simple as many assume. (For more on this see overleaf.) It is also an aspect of personal protective equipment where there is much misinformation and inappropriate advice. The result can be damage to workers' health, with all the costs that this entails, as well as frequently unnecessary expenditure on gloves that are really not fulfilling the employer's expectations.

No matter how carefully we design and operate our skin management system, it is not possible to guarantee that there will never be a situation where someone develops what may appear to be an occupational skin problem. What is then essential is that this is correctly investigated, the true cause (or causes) established, and an appropriate management strategy developed and implemented.

As the diagram shows, this is not as simple and many assume. It is easy to reach a conclusion that may lead to incorrect action in terms of treatment and management which can actually exacerbate the problem.

To correctly identify the cause - or frequently causes - requires a full assessment of both occupational and non-occupational factors, frequently the support of a medical specialist - usually a consultant dermatologist specialising in contact dermatitis - and a detailed evaluation of both the clinical and environmental issues that can affect the conclusions and subsequent management.

How confident are you.....

.....that the way you manage workplace skin exposure is actually up to date and correct? It is our experience that far too many believe that they have it right, whereas in reality their workforce is at significant risk of damage to health. This is one aspect of health and safety where there are many myths and misconceptions which, whilst appearing plausible, do not reflect reality.

For example:

If your skin exposure risk assessment is based on the safety data sheet, then there is a very real possibility that it is wrong!

If you believe that as airborne exposure to a sensitiser is below the WEL then your workforce is safe - you may be wrong!

Chris Packham needs little introduction. He’s the leading UK specialist in occupational risk assessment relating to skin hazards and techniques to measure skin condition. He has over thirty years’ experience in the interaction between the working environment and skin, and his background in engineering gives him a unique perspective of the subject. A Practical Guide to Occupational Skin Management is not just a health surveillance guide − it is relevant for many different roles including health and safety, occupational health and occupational hygiene. This is not an academic or medical textbook − Chris’ intention was to produce a practical guide for anyone aiming to minimise the risk of skin damage in the workplace. The book is designed so that individual sections can be read as stand-alone chapters − there is no need to read the whole book if you are only concerned with, for example, risk assessment. While this means that there is a certain amount of overlap of information, it makes the book easy to read as you don’t need to refer to other parts of the book while reading to understand a particular section. There are 15 sections covering subjects such as the skin and working environment, structure and function of the skin, potential problems, risk assessments, control measures, health surveillance and management of skin conditions. The section on ‘what can go wrong and why’ is particularly helpful. This, together with the explanation of the role of the skin in absorbing hazards, helps explain why risk assessment and control measures are so important. Health surveillance itself forms a small section of the overall book − as it should if the health and safety hierarchy of elimination, substitution, engineering controls and administrative controls are appropriately used to reduce exposure and ultimately a reduction in reliance on PPE. This section may be of most benefit to occupational health practitioners but many, particularly those new to health surveillance, would also gain much from the earlier sections. The book is written in Chris’ engaging and informative style; it is very easy to read, and even the complex anatomy and physiology is explained clearly. Although there are fewer references than a medical text, the key references are provided for those who would like some additional reading. In conclusion this is an excellent book which I would recommend adding to your library.
